Dear Sir,

I wish to enroll myself as an Ordinary Member of the Burmese Buddhist Temple (BBT) for Year 2024. I will make the payment of S$ 24.00 being the subscriptions of membership of the current calendar year.

As an ordinary member, I hereby allow my children to participate in the BBT Sunday School and activities organized by Burmese Buddhist Temple.

I also agree that: -

(a) I shall not hold Burmese Buddhist Temple, their appointed staff, officials and/or agents responsible for any loss of properties, mishaps, injuries or loss of life that may occur in the course of, or as a result of participating in the above activity; and

(b) I shall indemnify Burmese Buddhist Temple, their appointed staff, officials and/or agents against any actions, proceedings, liabilities, claims, damages and expenses by any party howsoever arising out of or in connection with the above said activity.

(c) Burmese Buddhist Temple, their appointed staff, officials and/or agents to collect my personal information (in the following registration form) for updating me about Burmese Buddhist Temple and its related activities, as well as statistics purpose.

I agree to abide by the rules and regulations as set out in the Constitution and any additions or amendments thereof, which may be introduced from time to time.
